Some posts on the Apple forums suggest you can restart
the MIDIServer if you shut down all applications that have been
using MIDI devices.

When it crashes it becomes what us geeks call a "zombie process" and
it doesn't completely die until you terminate all the other
processes that were communicating with it.

So you'll have to bring down Live, MIDIPipe, and MidiMonitor,
pause for reflection, then bring them back up.

As to why this happens I can't say.  MOTU is usually pretty good about
Mac drivers so the first place I'd look is MIDIPipe.  Is it doing
something you can live without?
