Hah!  It's the product of many mistakes in the past!

"Experience is the facility that lets you recognize your
mistakes after you've made them again..."

Generally, if it can go wrong it will.  (This is not
Murphy's Law, which is "If there's a wrong way to do
something, someone will do it.")  So you just have
to have a list of all the ways that something can 
go wrong and you're set!
