You can get Hubi's MIDI Joystick program (the people who brought you Hubi's MIDI Loopback) that uses a regular joystick to send MIDI messages.

I have no idea if it works with XP or Vista though. 

There's a VST called MIJOY that does something similar.

It would it be fun to bust out a joystick during a live performance... MORE IMPORTANTLY it would be amazing to have an audience member be able to modulate your performance in some way... just imagine the possibilities, the numerous parameters of the numerious midi controlled devices!  wow!
